HIV-FRAT: A Breakthrough Tool for Safer, Smarter HIV Care

Every day, people living with HIV (PLWH) face silent threats—not just from the virus, but from complications that come with aging, medications, and long-term health conditions. One such silent threat? Falls. Yes—something as simple as a stumble can have devastating effects on someone with a weakened immune system or multiple health issues.
That’s where HIV-FRAT comes in. And it’s changing the game.
🧠 What Is HIV-FRAT?
HIV-FRAT stands for HIV Fall Risk Assessment Tool — a simple, science-backed checklist that helps doctors and healthcare workers predict the risk of falling in people living with HIV.
It was developed by Professor Sam Chidi Ibeneme and his team at the David Umahi Federal University of Health Sciences, Uburu — and it’s the first of its kind from Nigeria, backed by global scientific standards through a systematic review and meta-analysis.
🩺 Why Does It Matter?
People with HIV are living longer, thanks to modern medicine — but as they age, they are more likely to experience:
• Side effects from HIV medications,
• Mental health challenges like depression,
• Nerve issues, muscle weakness, and
• A mix of other chronic diseases.
These can all increase the risk of falling—leading to broken bones, hospitalizations, or even death.
But until now, there was no easy tool for health workers to identify who was most at risk. HIV-FRAT fills that gap.
📋 How Does HIV-FRAT Work?
It’s incredibly easy to use — like a checklist.
It asks 9 simple questions based on 9 proven risk factors, such as:
Risk Factor Score (Risk Weight)
Cannabis use 3.0
Neurocognitive adverse effects 2.3
Cognitive impairment 2.3
Frailty 2.0
Hypertension (high blood pressure) 1.8
Depression 1.6
Use of Stavudine (an HIV drug) 1.5
Polypharmacy (too many meds) 1.2
Peripheral neuropathy (nerve pain) 1.1
Each “Yes” gets the score. Total the scores.
The higher the number, the higher the risk.

8 – 12.7 Moderate Risk Review medications, provide counseling
12.8 and above High Risk Refer to physiotherapy, start interventions
